Only two English players will start the match, with no Scots in the team either.
After the mauling received by the front row in Durban, the Lions have dropped hooker Lee Mears and tight-head Phil Vickery in favour of Welsh duo Matthew Rees and heavyweight Adam Jones.
Additional beef has been added to the pack with Simon Shaw coming in at lock in place of Alan Wyn-Jones. Tight-head Andrew Sheridan takes a place on the bench should more brute force be required.
Wing Ugo Monye also loses his place to Irishman Luke Fitzgerald. The injured Lee Byrne is replaced at fullback by Rob Kearney.
South Africa welcome back Shalk Burger to their back row but are otherwise unchanged from the side fielded in the first test.
