Google "URLs not followed"

My web site has been indexed without problems with Google for over a year. Now they show Warnings on the Google Webmaster Tools Siremap Details:-
"URLs not followed
When we tested a sample of the URLs from your Sitemap, we found that some URLs were not accessible to Googlebot because they contained too many redirects. Please change the URLs in your Sitemap that redirect and replace them with the destination URL (the redirect target). All valid URLs will still be submitted. "

My web site has links to Car Hire companies, Weather sites, etc - frankly I don't understand what is wrong or what I should do.

Can anyone explain in simple language??

Web site is "Commercial URL removed"

I'm guessing some of the links to other sites are actually pointing at pages that have been changed and are now just forwarding the user on to the new page. Google is telling you that you need to change these links to point at the new pages address not the old one.
